Need help asap, no incoming mail, no bad event logs.

I have internal mail, outgoing mail, outgoing in messages say they have been
relayed but don't get to the client. But clients can send out and to each
other.
No event logs to say anything, turned up logging to 4th level and still
nothing? Reinstalled SMTP, checked all services, ran scan disk. Can telnet on
port 25 to server from outside, so not firewall problems.

Mail just wont get from the outside of the server to the mailbox of the
server. Please help been working on this for 5 hours and must have it up.

Next option is reinstall of exchange will this delete the mail store?

Have you checked your MX Record?  Sounds like the ISP may have changed DNS
Servers.  I would call the ISP that handles your External DNS to insure
nothing has changed.
