User receives e-mail addressed to another mailbox

I have a client where user A occassionally receives spam addressed to user B
(both on the same exchange 2003 server). I figured revealing the headers
would show  user A's email address, but it doesn't, it only shows it
addressed to user B. I'm afraid this might be a sign of something more
sinister, such as one of these PCs has been compromised, under the control of
a spammer. Or worse, the Exchange server has been compromised. Anyone chased
this one down?

thanks!
